COVID-19 Vaccine Providers

Individuals 6 months of age and older currently qualify to receive the COVID-19 vaccine. For those interested in scheduling their appointment or scheduling an appointment for their child, vaccine providers are listed below. 

Parent or guardian consent will be required for individuals younger than 18 years old to receive the COVID-19 vaccine.
